Some people have problems with their legs.
直訳すると何人かの人々は足に問題を持っていると言う意味になります。このwithは〜にという意味で使われています。

中学校の英語でよく
something wrong with〜で〜の調子が悪いという文を見たことがあると思うのですが、これは直訳すると〜になにか問題があるとなります。それが意訳されて〜の調子が悪いとなったんです。
なのでこのwithも〜にという意味で使われています。
