オレンジマーカーしている箇所なのですが、
1️⃣の（4）の答えは「He practices the piano on both Tuesday or Friday 」となっており、onが関係接続詞の前に来ているのですが、3️⃣の（1）では答えが「She’s gone to either to the department or to the hospital 」となっておりonが関係接続詞の中に入っています。なんでですか？

なぜなら、等位接続詞 and/or は前後に[同じ働きをする表現]を置き、それを結び付ける役割があるからです。
He practices the piano on both [Tuesday] and [Friday]. でも He practices the piano both [on Tuesday] and [on Friday]. でもＯＫ。
She’s gone either [to the department] or [to the hospital]. でも She’s gone to either [the department] or [the hospital]. でもＯＫ。
