have + 過去分詞じゃなくて、be + 過去分詞でも完了の意味で使うんですよ。
だから、辞書ではdoneを形容詞として扱って「終わって」とかの意味があると説明しています。

ここでは
doctor (being) doneということでいいと思います。
この場合だとbeingが分詞構文でdoctorが意味上の主語
そして、その続きの
her hand or patient's (being) 〜 on the doorknob
とも表現が揃います。これもbeingが分詞構文、その前までが意味上の主語です。

もしくは
doctor (is) done 時制がわかりませんが
her hand or patient's (being) 〜 on the doorknobが分詞構文の挿入
と考えてもいいです
