いえ、これは接続詞のwhere（"〜のところで"）です。

whenに疑問詞と関係副詞と接続詞の用法があるのと同じで(辞書には他にもいくつかありますが)
whereにも疑問詞・関係副詞ほどは使われないかもしれないけど、接続詞の用法があります。

んー、コメントをいま拝見してもう一度確認してみましたが、このwhereを関係副詞と考えるのはちょっと無理あるんじゃないかなぁと思いました。

基本的な関係詞の用法では
whereの節の前に先行詞（=名詞）がありますよね。
今回はwhereの前は動詞のhappenedです
あるいは、
主節の主語に、長い関係詞の節がつくときには、
関係詞節だけ後ろに回ったりする場合もあります
今回をその事例として考えると
where〜の節の先行詞がaccidentってことになる
つまり
点字ブロックがまだ置かれていない（という）アクシデントが時折発生した
という意味になっちゃう。
これは話の流れ的に変だと思います

whereを接続詞と考えると
点字ブロックがまだ置かれていなかったところでは、時折アクシデントが起こった
と読めます。
こちらの方が自然じゃないかと思います。
