This gives the answers but I don't believe giving answers is the best answer for homework help. The point of helping is to guide someone to come to an answer by themselves.

The sentence that "Have you ever gone to" may be a little bit odd.
You know "gone" means that you got somewhere and stay there. It probably focuses on a result.
So the question that "Have you ever gone to ~" can be wrong in that sentence, isn't it? Um
The answer which I think is correct is "Have you ever BEEN to ~?"
Am I correct? Um u know I'm not a native English speaker so I'm not really sure wether I'm correct or not. So I hope to hear from you :)

Actually been to is better but they use 行くso I translated it as gone. Been to would me iいる

In native English in speaking some people do say
Have you ever gone to ...? But that is indeed wrong use. You will find that even native speakers do not always follow grammar rules when we speak.
