1.Ken does look happy today.
助動詞doは直後の動詞(今回はlook)を強調します。
助動詞doは、変形と考えられるものに、
does(三人称単数に続き、時制が現在)
did(主語に関係なく、時制が過去)
があります。

簡単に考えると、
「その文を否定文にしてからnotを外す」ことでできあがります。
→Ken doesn't look happy today.
→Ken does look happy today.　完成！

2.Nancy does not take after her mother at all.
notを強調する表現に、「not ～ at all」があります。
この表現では、at allを主に「文尾」につけます。
ですので、ピリオド(.)の前にat all.

3.David is not in the least afraid of dogs.
または、David is not afraid of dogs in the least.
どこかで見た例文では、上の文に見覚えがありますが、
in the leastが、
①notの直後
②文尾、文の最後
の例がどちらも見られますし、特に目立った違いはないようです。どっちでもOK(ただし、not ～ at allは文尾がいいみたい)
同じような表現に、not ～ in the slightest、not ～ in any wayがあるので調べてみてね！

4.What on earth are you talking about?
on earthは、WhatやWhenなどの疑問詞を強調します。
訳しかたは、「一体全体～」とかがあります。
これは、「疑問詞の直後」において、What on earthのかたまりにしてくださいね。

ちなみに、こちらの同様の表現には、What in the worldがあります。onとin間違えないようにね！
