thatを使わないのなら、The woman "whom" you met in the hospital is my aunt.
になると思います。
The woman is my aunt.と、
You met "her" in the hospital.
という英文を合わせた形ですからね。herは目的格なので、目的格の関係代名詞はwhomです。

最近は口語ではwhomもwhoとするのが一般的らしいので、whoも間違いではないのですが。
文法的にはwhomの方が正しいですし、thatで言い換えができるので、確実なthatの方が使われているのだと思います。
